Director of Social Services

Green Brook Township, NJ

The Director of Social Services will provide medically related social services to attain or maintain the highest practicable physical, mental, and psycho-social wellbeing of each patient.

Responsibilities:

  • Oversee and supervise the social services department, ensuring the delivery of high-quality services to clients
  • Develop and implement case management services to all residents
  • Maintain contact with family (with resident's permission) to report on changes in health, current goals, discharge planning, and encouragement to participate in care planning
  • Maintain progress notes under federal and state guidelines for Long Term Care.
  • Ensure compliance with all regulatory requirements and standards in the provision of social services
  • Facilitate the building of relationships between residents and staff by teaching staff how to understand and support residents' individual needs.
  • Provide leadership and guidance to staff, including training, performance management, and professional development

Skills:

-Strong supervisory skills, with the ability to lead a team effectively

- Extensive knowledge of social work principles, practices, and ethics

- Administrative skills, including budgeting, program management, and resource allocation

- Experience working with individuals with developmental disabilities and behavioral health issues

- Excellent communication and interpersonal skills for relationship management with clients, staff, and external stakeholders

Requirements:

  • LCSW or CSW is required. Must be a licensed social worker.
  • BA from an approved Social Work school is required.
  • Subacute and geriatric experience
  • Minimum one years’ experience with seniors. In Supervisory capacity in a hospital, skilled nursing care facility, or other related medical facilities.
